What to Bring to Your Immigration Medical Exam

Required Documents

Form I-693: You must bring Form I-693 with the applicant sections completed prior to your appointment.

Photo Identification: A valid government-issued photo ID, such as a passport, driver’s license, or official state/government ID.

Additional Documents to Bring

Vaccination Records: Bring any vaccination records you have, including international records. If you do not have them, we can help determine which vaccinations you need.

Health Insurance Card: If you have health insurance, please bring your card so that laboratory tests can be billed through your insurance. If you do not have insurance or prefer not to use it, self-pay options for lab services are available on-site.

A-Number (USCIS Number): If you have a USCIS A-Number (starts with ā€œAā€ followed by 8 or 9 digits, found on USCIS correspondence), please bring it. If you do not have one, it is not required.

No Laboratory Tests Before Your First Visit

Your required testing will be determined as part of your immigration medical examination. After reviewing your medical history, vaccination records, and USCIS requirements, our team will explain which laboratory tests or vaccinations you may need and help coordinate them as efficiently as possible.
