What to Bring to Your Immigration Medical Exam
Bring a valid government-issued photo ID, current photo ID, such as a passport, driver’s license, or another valid government-issued identification document.
Helpful Documents to Bring, If You Have Them
Previous Form I-693
If you have a previously completed Form I-693, please bring it. A blank form is not required.
A-Number, if available
Bring your USCIS A-Number, which starts with āAā followed by 8 or 9 digits. You can find it on USCIS correspondence. If you donāt have one, thatās okay, itās not required.
Vaccination Records
Bring any vaccination records you have, including records from another country. If you donāt have them, we can help determine what you may need.
No Laboratory Tests Before Your First Visit
Your required testing will be determined as part of your immigration medical examination. After reviewing your medical history, vaccination records, and USCIS requirements, our team will explain which laboratory tests or vaccinations you may need and help coordinate them as efficiently as possible.
